Claim Reporting

Please report all claims to: claims@centrex.com

 

  • The insured’s first notice of the claim or incident should include the following information:

 

  • The date, time and location of the incident

 

  • The distance from the insured’s establishment to the location of the incident

 

  • A detailed description of the incident

 

  • A description of the nature of the injuries, if any

  • Save all videos taken at the time of the incident

 

  • Name, home address and home phone number of the insured’s management

 

  • Names, home addresses and home phone numbers of all of the insured’s employee staff working at the time of the incident, including the identity of all alcohol serving or selling employees who may have served a patron involved in the claim or incident

 

  • The identity of all witnesses who are not employees of the insured

 

  • The identity of any vendors potentially involved in the incident (security service, etc.) including a copy of all relevant contracts with such vendors

 

  • Copy of any document(s) confirming service to the parties involved in the incident (receipts, credit card slips, etc.)

 

  • Copy of any security camera tape that may be relevant to the claim or incident

 

  • Copies of documentation confirming that all alcohol serving or selling employees have had certified alcohol awareness training (i.e., Centrex Online Training, TIPS, TAM, BEST, RAMP, TABC-Certified, Serve-Safe, Learn2Serve, etc.)

 

  • Copies of all suit papers received by the insured regarding the claim or incident

 

  • Copies of any newspaper articles about the claim or incident

 

  • After the first report of a claim or incident, the insured should report all of the following subsequent developments to their retail agent for forwarding to the insurer:

 

  • Copies of all suit papers, discovery notices, disclosure notices, and other legal papers received by the insured regarding the claim or incident

 

  • Any additional relevant information regarding the claim or incident that comes to the insured’s attention

 

  • The identity of any new witnesses that come to the insured’s attention
